Music Department Event Form

Complete this form to request venue access, equipment and/or support.  Please note that submitting this form is not a guarantee of access, equipment or support.  Your request will be evaluated by CCAPS and they will reach out with questions or concerns.  If necessary, a production meeting may be required.  Please review, completed, and submit this form no less than 30 days prior to your event.

Once this form is submitted, you will receive a request asking for technical and other details.

Set Ticket Prices

There are set ticket prices for events as follows:

  • Adults:  $10
  • Seniors (65 years and older), Non-CCA Faculty and Staff:  $8
  • USU Students and Youth (grades 3-12):  FREE
  • CCA Faculty and Staff:  1 FREE ticket

Tickets will go on sale one (1) week after this event has been confirmed.

Marketing Information

If marketing is needed, your initial marketing meeting must take place with Whitney Schulte at least 8 weeks before your event.

CCAPS is not responsible for the effectivenes, timeliness, or cost of any marketing materials ordered less than 4 weeks before the event.

Non-CCAPS created marketing materials must be approved by CCAPS Marketing to ensure compliance with Utah State University and Caine College of the Arts branding practices.
